Abstract
이 연구의 목적은 Cone-beam형 전산화 단층촬영술 영상을 이용하여 정중과잉치의 3차원적인 특성들에 대해 조사하고 이 특성들과발거 시 마취 방법 및 합병증에 영향을 미치는 요인들에 대해 분석하는 것이다. 이를 위해 2017년부터 2019년까지 서울대학교 치과병원 소아치과에서 과잉치를 발거한 환자 452명의 총 602개의 정중과잉치를 평가하였다.
지난 20년간 연도별 전체 환자 중 정중과잉치 환자수의 비율이 점차 증가하는 경향을 보였다. 정중과잉치 방향 중 순-구개 방향의수평위가 근원심 방향의 수평위보다 약 3.2배 많이 관찰되었으며 순-구개 방향의 수평위 중 치근이 순측을 향하는 경우가 전체 수평위 중에서 가장 높게 나타났다. 정중과잉치의 시상면에의 상하 위치는 대부분이 인접 영구치의 치경부 측(37.0%)에서 가장 많이 관찰되었고 순-구개측의 매복 정도를 관찰한 결과 근-구개측이 원-구개측의 약 3.83배 정도 더 높게 관찰되었다. 대부분의 정중과잉치(98.5%)가 CBCT상에서 인접 영구치와 접촉해 있는 것으로 나타났다. 정중과잉치 중 46.2%에서 만곡된 치근이 관찰되었고, 치근이 만곡된 정중과잉치 중 치근의 치근단측 에서의 만곡이 치근의 치경부측에 비해 약 1.83배 높게 나타났다. 소아환자의 정중과잉치 발거시 사용한 마취 방법 중에서는 전신마취(72.6%)를 시행하여 정중과잉치를 발거한 경우가 가장 많았다. 환아의 연령, 시상면에서의 상하 위치, 부작용 유무가 전신마취 시행 여부에 유의한 영향을 미치는 요인으로 확인되었고 정중과잉치의 방향 및 시상면에서의 상하위치가 합병증에 영향을 미치는 것으로 나타났다.
이번 연구의 결과는 정중과잉치에 대한 이해를 도와 정확한 진단 및 적절한 치료계획을 수립하는데 유용한 정보를 제공할 수 있을것으로 사료된다.
The purpose of this study was to investigate the three-dimensional characteristics of mesiodens using Cone-beam Computed Tomography(CBCT) and analyze the factors affecting complications and anesthetic methods of extraction. This study evaluated 602 mesiodens of 452 patients who underwent extraction of mesiodens at the department of Pediatric Dentistry in Seoul National University Dental Hospital between 2017 and 2019.
The ratio of mesiodens patients over total patient per year was gradually increased over the past 20 years. Mesiodens with labio-palatally horizontal direction while root directing labial were the most common among the mesiodens with horizontal direction. Mesiodens were the most common at the cervical side of the adjacent teeth(37.0%) and mesiodens located in the near-palatal side were observed about 3.83 times higher than the far-palatal side. Most of the mesiodens(82.1%) were in contact with adjacent permanent teeth on all three sides of the CBCT and 46.2% of mesiodens had curved roots. The patient’s age, vertical position, presence of complications, and proximity showed a significant difference in the selection of general anesthesia among anesthetic methods. The direction and vertical position of mesiodens had a significant effect on complications.
These results provide a better understanding of mesiodens for establishing an accurate diagnosis and treatment plan.
